  • what merchandise you want, including the size
  • where to ship it
  • the type of credit card (Visa/MasterCard only), the credit card number, the expiry date, the 3-digit CVV2 (You know what this is, right?), and the name on the card. (Speak slowly and carefully, please, and spell things where necessary.)
  • your phone number or email address (again, slow and careful) so we can contact you to confirm the order

• If you want to order by fax, write out what you want on a single sheet of paper. Write neatly (or better, print it out) and make sure you include:
  • what merchandise you want, including the size
  • where to ship it
